How do I make an appointment for an evaluation?

To make an appointment, please call the office at (720) 378-6670. You will be asked to provide personal and insurance information (if applicable). At the same time, feel free to download our forms on the General Information page.

What happens during an evaluation?

The evaluation will be usually be about 1-2 hours.
Prior to the evaluation, patients complete the intake forms, which provide information regarding their concerns, the medical history, current concerns, as well as, the review of our policies. These forms can be found on the website under General Information, or will be given to you when you schedule the appointment. We ask that you please arrive 10 minutes early to ensure all necessary paperwork is complete. If the patient is receiving other medical or rehabilitative services, please bring a copy of the reports.
During the evaluation, the patient's medical history is carefully reviewed. Patients and their family members/caregivers (if applicable) are interviewed regarding their concerns. This information helps the therapist identify areas to evaluate more closely. A variety of methods, including formal and informal tests, observation, patient/caregiver interview, will be used to evaluate the patient's abilities. Selection of testing methods is based on the patient's individual needs.
Following the evaluation, initial results of the evaluation and recommendations will be reviewed with you. A written report detailing evaluation results and recommendations will be given to you as well as faxed or electronically sent to the primary care physician.

What happens in a therapy session?

Treatment plans and goals are implemented based upon the patient's individual needs. Treatment goals and strategies are generally implemented on a 1:1 delivery basis. Family members or caregivers may also be involved in the treatment sessions.

How long is a therapy session?

We offer 30, 45, and 60 minute therapy sessions, depending on the needs of the patient. We respect your time as well as our own, and therefore, expect to start and end therapy at the designated scheduled times. The clinician will brief the patient, and family member/caregivers (if applicable) regarding the session and the recommended homework (if appropriate) the last few minutes of the session.

What do parents/siblings do during therapy?

Parents and siblings are encouraged to participate in most therapy sessions. We believe that this allows everyone in the family to understand what the goals are and how to best support learning after the session has ended. If the therapist needs the patient to participate in an activity alone, a waiting room is available for additional family.

Can you bill my insurance?

In most situations, we can bill your primary insurance. We are out-of-network providers for MOST insurance companies. We are willing to work with your insurance company to pursue authorization & payment. We can not guarantee insurance will cover therapy services.
